導師服務滿意度評量：
(一) 1142學期「導師服務滿意度評量」調查作業開放填寫時間為115年05月11日(一)起至115年05月29日(五)止(第12週至第14週)，請同學於截止日期前儘速抽空上網填寫。
The “Class Advisor Service Satisfaction Survey” for Semester 114-2 will be open from Monday, May 11, 2026 to Friday, May 29, 2026 (Week 12 to Week 14). Please take some time to complete the questionnaire before the deadline.
填寫路徑：弘光首頁→在校學生→學生資訊系統 →學務→導師服務滿意度評量。
(二) 問卷送出後，系統僅提供一次重新修改「評量」資料之機會，請同學於送出前務必再次確認填答內容，並審慎作答。
After submitting the questionnaire, the system only allows one opportunity to revise your evaluation responses. Please carefully review your answers before submission.
(三) 問卷開放填寫期間，導師僅能查詢班級整體問卷「填答率」及「完成人數」，無法透過任何方式得知個別學生是否已完成問卷。系統亦不提供相關查詢功能，因此導師無法得知學生個人對導師評量之成績、內容或意見，以保障學生填答之匿名性與公平性。
During the survey period, advisors can only view the overall response rate and the number of completed questionnaires for the class. They cannot identify whether individual students have completed the survey through any means. The system also does not provide access to individual responses, scores, comments, or opinions regarding advisor evaluations, in order to protect students’ anonymity and ensure fairness.
